The results were impressive:
Average 'excess' length of hospital stay for 'complex' patients was reduced from 19 to 7
Hospital discharge assessments reduced by 81%, giving staff more time to plan the care
Delayed Transfers of Care out of hospital awaiting 'public funding decision' reduced to zero
Zero complaints from patients and relatives
Reduced number of Continuing Health Care packages on hospital discharge from 14 to 2
NHS savings of £562,000 in a 7 month period in the 1st evaluation and savings of £1.3m in a 4 month period in the 2nd evaluation
Minimal impact on social care referrals
Further evaluation in 2019 of over 3,000 patients in three ‘5Q Care Test’ adopter sites reported a 99.2% reliability in determining whether people needed social care on discharge from hospital.
